Community Visiting

The Clearwater & District Hospice Society trained volunteers are available to go out into the community to provide support, respite care, and companionship to clients in the community living with life-limiting illnesses.

Referrals

At your Doctor’s Office: have your doctor fill out the referral form and send it to the hospice email address.

email clearwaterhospice91@gmail.com with your request to have the hospice coordinator contact you.

The hospice coordinator will complete an assessment followed by customizing a volunteer visit to fit the family schedule. Volunteers visit clients in their homes, long-term care settings or in the hospital. The volunteers provide respite, support, and companionship for those living with a life-limiting illness. This allows the caregivers to have some much-needed rest, leave the home to visit a friend, grocery shop, engage in personal care treatments or have an opportunity to work in the yard.

Advanced Care Planning

Advanced care planning is where capable adults share their beliefs, values, and wishes for health care decisions with close family or friends and health care providers in advance of a time when they may be incapable of deciding for themselves.

Patients need to communicate verbally or in writing what treatments are important to them and what they would and would not accept. They should also choose a substitute decision maker who will speak and act on their behalf and honour their wishes.

Palliative Social Worker

Interior health has a visiting social worker. Contact your physician to be referred to the social worker.

Wind Phone

The Phone of the Wind is an unconnected telephone in a peaceful location on the trail behind the Dr. Helmcken Memorial Hospital where the Hospice Society’s Palliative care room is located. The Clearwater Hospice Society set up the phone for grieving family members to channel their grief by speaking with their loved ones who have died.
